Must say I'm with Pedro on that one.
Have been working on MEP modelling for the last 3 years now.
There a valid reason for MOST of the MEP tools to not automatically create penetrations.
This is because the MEP engineers place the objects/components.
BUT, ...... It is typical/normal for the architects and structural engineers to have to review these penetrations, holes, openings, trenches and the like, in the walls and floors, because they are 'their' walls and floors. They need to 'approve' these before the are allowed to be incorporated in to the design.
There is typically a process for this and generally speaking it is 'quite a big deal'

There is a whole series of excercises/reviews (typically called Builders Work Holes in connection ...... BWH) regarding this.
So on the whole I would have to say no, you don't want MEP objects to automatically create holes/openings
